Scottsboro City Board of Education 305 South Scott Street Scottsboro, Alabama 35768 October 26, 2010 Dear Parent/Guardian: Because of compliance regulations for the Federal Impact Aid Program we have to re-survey our student population. We appreciate your response on the previous form, and we are sorry for any inconvenience this may cause. Please complete the new form and return to your child's teacher no later than November 9, 2010. Scottsboro City Schools is committed to securing all monies available to our students. The survey can also be found on the district website, www.scottsboroschools.net. From district website, click on "District News" and then click on "Federal Impact Aid". You can print the survey from the website and complete it. Scottsboro City Schools are entitled to certain federal monies if mother, father, or guardian of a student is employed on certain Federal Properties, is on ACTIVE DUTY in the UNIFORMED SERVICES or RESERVES, or live in a Federal Housing Project. To collect this money, Scottsboro City Schools must be able to substantiate our claim with information requested on the attached survey. Please complete the attached Parent-Student Survey for each child. Section I should be completed in its entirety and Sections II, III, and IV as appropriate on the form. - SECTION I: One form should be filled out for each student that attends Scottsboro City Schools in drades PreK-12. Please complete every blank in Section I to include siblings at other schools.
- SECTION II: If the student and family live on Federal Property (Military Housing/US Housing Authority), check "yes" and check box beside property listed in Section II. If "no" is checked go to Section IV
- SECTION III: If parent/guardian is employed by Federal Government or works on Federal Property, fill in Section III. If parent/guardian checked yes, please list the name of parent/guardian and address of the employer. (List both parent's names if both are employed on Federal Property).
- SECTION IV: If either parent/guardian is on ACTIVE DUTY in the UNIFORMED SERVICES or RESERVES, check "yes" and fill in name, branch of service, and rank of the parent/guardian. If the parent/guardian is not on active duty, check "No."
- SECTION V: In order for you to be counted, please sign and date the form on or after October 26, 2010.
- If none of the questions apply to you, fill in School, Teacher, and Student Information and mark "NO" by all Questions. Sign at the bottom of the page and return to your child's school so they will know that you have seen the Federal Impact Survey Form and it does not apply to you.
The information requested is the minimum information required by the Federal Government in order to receive money for operating our schools. Your help is needed to obtain these necessary funds. Please fill out the survey form, sign, and date as of October 26, 2010 or later, and have your child return it to the school as soon as completed.
